The movie is about the future. It seems that in this world, the humanity exhausts all natural resources, scientists create genetically modified orchids as a new energy source and develop a surgery that makes everyone "Pretty" at age 16. This procedure eliminates differences and keeps society peaceful. Tally and her friend Peris are "Uglies," but Peris, being older, undergoes the surgery first. When Tally sneaks out to see him, she is shocked by his new appearance and personality.
Back at the dorms, Tally befriends Shay, a rebellious Ugly who talks about "The Smoke," a place where people live freely in nature. Shay invites Tally to join her and escape before the surgery, but Tally is too scared to leave. On her 16th birthday, Tally is denied the procedure until she helps Dr. Cable, the head of the operation, find Shay. Tally is told that Shay is in danger from David, the leader of The Smoke, who is supposedly building a weapon to wipe out The City.
Tally reluctantly agrees and is sent to The Smoke as a spy. There, she reunites with Shay and meets David, learning the truth about the surgery: it alters the brain to make people compliant. David’s parents, former scientists, fled the city after discovering this and have been working on a cure. Tally also learns that the orchid energy source is harmful to nature, which is why The Smoke rebels burn them. As she grows close to David, Tally decides to renounce the system and throws away her tracker.
Will Tally able to save Shay or will she betray David and all his friends?
